Pre-Operative Care Before Rhinoplasty

Careful adherence to the following instructions is essential to ensure safety, reduce complications, and achieve the best possible surgical outcome.

1. Required Medical Consultations

If you are over 40 years old or have a history of:

  • Heart disease
  • High blood pressure
  • Palpitations
  • Blood clotting disorders or previous thrombosis

A cardiology consultation is mandatory.

2. Full Disclosure of Medical History

If you have any medical condition, including:

  • Thyroid disorders
  • Diabetes
  • Rheumatological diseases
  • Neurological conditions (including seizures)
  • Psychiatric conditions (depression, OCD, etc.)
  • Kidney or liver disease
  • Or any other underlying condition

You must inform the doctor in advance.

If necessary, specialist consultations will be arranged and Pre-operative preparation, Post-operative care and Medication adjustments will be planned accordingly.

Please bring all consultation reports on the day of surgery.

3. Medication History

Please inform us about all medications, including Herbal medicines, Supplements and Vitamins, So that the timing of discontinuation before surgery and resumption after surgery can be determined safely.

4. Required Documents on Surgery Day

Please bring all of the following:

  • Blood test results
  • CT scan
  • Professional photography (studio photos)
  • Consultation reports
  • Identification documents:
    – Iranian patients: National ID card or birth certificate
    – Non-Iranian patients: Valid passport
    – Afghan nationals: Amayesh card

5. Legal Consent on Surgery Day

Patients under 18 years old must be accompanied by one of the following:

  • Father
  • Spouse
  • Legal guardian (with official guardianship document)
  • Paternal grandfather

Patients 18 years and older:
(Spouse presence is required for legal consent signature.)

6. Blood Tests

Prescribed blood tests must be completed no more than two days before the surgery and submitted to the clinic prior to the procedure.

7. Shower the Night Before Surgery

The night before surgery:

  • Take a full shower
  • Ensure hair and face are completely clean
  • Do not apply creams, lotions, or body oils

8. Neck Pillow

Prepare a wide neck pillow. It is necessary for sleeping and resting for 2–3 weeks after surgery.

9. Nail Polish & Artificial Nails

At least one index finger on each hand must be free of nail polish and free of artificial nails or gel (for monitoring vital signs during anesthesia).

10. Clothing on Surgery Day

Wear comfortable clothing. Preferably button-up or wide or open neckline, to avoid pressure on the nose after discharge.

11. Makeup & Eyelashes

Female patients:

  • Must come without makeup
  • Face should be completely clean
  • Avoid artificial eyelashes (to reduce infection risk)

12. Shaving for Male Patients

Male patients must:

  • have beard and mustache completely
  • Shave the right lower leg (for electrocautery grounding pad placement)
  • If rib cartilage harvesting is possible: Chest hair must also be shaved

13. Writing Down Your Expectations

If you are worried about forgetting your aesthetic goals, write them down and bring the note to the operating room. Discuss them during the final pre-operative consultation.

14. Travel After Discharge

If you plan air travel or ground travel after discharge, inform the doctor on surgery day so a flight clearance letter can be issued if needed and travel precautions can be explained.

15. Admission Time & Patience

The announced admission date and time are very important. Please plan transportation to arrive on time to avoid delays.

After admission:
Be patient during preparation and transfer to the operating room. Previous surgeries may take longer. The same care and time will be dedicated to your surgery.

16. Fasting

From 12:00 midnight before surgery,  do not eat or drink anything. If thirsty before 6:00 AM, only plain water is allowed.

17. Mental Calmness on Surgery Day

The most important factor on surgery day is your peace of min. Knowing that:

  • Surgery is performed in a specialized hospital
  • A professional medical team is present
  • You have received full pre-operative counseling

Enter the operating room calmly and trust the process.

18. Smoking, Hookah & Vaping

Stop using:

  • Cigarettes
  • Hookah
  • Vaping & nicotine

At least 3–4 weeks before surgery, as they impair wound healing, increase infection and bleeding risk and negatively affect final results.

19. Alcohol & Stimulants

Avoid for 7–10 days before surgery:

  • Alcohol
  • Energy drinks
  • Stimulant beverages

20. Blood-Thinning Medications

Medications such as:

  • Aspirin
  • Ibuprofen
  • Naproxen
  • Warfarin
  • Supplements (Omega-3, Vitamin E, Ginseng, Garlic)

Must be stopped only under medical supervision.

21. Cold & Infections

If you experience:

  • Cold or flu
  • Sore throat
  • Fever
  • Cough
  • Sinus or dental infection

Inform the clinic, as surgery may need to be postponed.

22. Menstrual Period

Surgery during menstruation is not prohibited. However, inform the doctor if you have heavy bleeding, Anemia or weakness.

23. Facial & Skin Treatments

Avoid the following at least 10 days before surgery:

  • Laser
  • Waxing
  • Facial treatments
  • Microneedling

24. Cold Sores (Herpes)

If you have an active cold sore on:

  • Lips
  • Around the nose
  • Inside the nose

Inform the doctor immediately.

Active herpes may increase infection risk and delay wound healing.

Surgery may be postponed or preventive medication prescribed.

  • Jewelry
  • Earrings

27. Sleep & Nutrition Before Surgery

The night before surgery:

  • Get adequate sleep
  • Eat a light meal
  • Stress is normal, but lack of sleep increases swelling and fatigue.

Post-Operative Care After Rhinoplasty

Following these instructions carefully is essential to reduce swelling, prevent complications, and achieve the best final result.

1. Rest, Sleep & Body Position

  • Get enough rest, but do not stay in bed all day during the first days.
  • Gentle walking at home or sitting with your head elevated helps reduce swelling.
  • For at least 1 month:
    – Sleep on your back
    – Keep your head elevated
    – Use a wide neck pillow
    – Avoid sleeping on your side or face.

2. Cold Compress

During the first 48 hours, apply cold compresses to the cheeks and around the eyes only, for 10–15 minutes each time. Do not place ice directly on the nose.

3. Medications

Take all prescribed medications (antibiotics, painkillers, sprays, ointments) exactly as instructed.
Do not stop, change, or add any medication without consultation.
Report any allergic reactions or unusual side effects immediately.

4. Nasal Care & Washing

Rinse the nose with saline solution as instructed.

After each rinse:

  • Use nasal spray (2 puffs)
  • Apply gentamicin ophthalmic ointment gently inside the nostrils and on incision areas.

5. Blowing the Nose & Facial Movements

Do not blow your nose for at least 3 weeks. When sneezing, coughing, yawning, or laughing, keep your mouth open and support your cheeks with both hands to avoid pressure on the nose.

6. Nutrition & Fluids

  • Follow a low-salt diet.
  • Avoid spicy foods, pickles, and excessive seasoning.
  • Fresh fruits and vegetables, protein-rich foods and fresh natural juices are recommended
  • Excessive intake of hot-nature foods, large amounts of pineapple or fatty foods
    may cause pimples under the cast and skin irritation.
  • Warm milk with honey and a small amount of turmeric may help reduce swelling.
  • Drink plenty of fluids, especially water, celery juice, and fresh juices.

7. Smoking & Nicotine

Smoking, hookah, and vaping are strictly prohibited for at least 4 months after surgery.

8. Showering

From 2 days after surgery:

  • Shower from the neck down
  • Avoid wetting the face and nasal cast

On the cast removal day:

  • If access to the clinic is short: full shower is allowed
  • If travel time is long: avoid wetting the cast (Prolonged moisture may damage nasal skin)

9. Sexual Activity

Sexual activity is allowed after cast removal.

10. Protecting the Nose & Glasses

Avoid any trauma to the nose during the first few months.
Sunglasses and prescription glasses are prohibited for six weeks. After that, only lightweight glasses may be used with the doctor’s approval. If necessary during the early period, a glasses holder should be used.

11. Exercise & Physical Activity

  • Light walking: allowed
  • Heavy workouts (weights, jumping, TRX): after 3 months
  • Swimming: after 6 months
  • Contact sports (football, volleyball): Use a medical nasal protector

12. Warning Signs

Contact Us Immediately If You Notice:

  • Persistent or heavy bleeding
  • Severe or increasing pain
  • Fever
  • Foul-smelling nasal discharge
  • Unusual redness, warmth, or swelling
  • Sudden worsening of swelling or pain

Care After Cast Removal

Cast removal does not mean the end of care — this phase is critical for shaping the final result.

1. Nasal Taping

  • Duration of taping is determined by the doctor, usually after the second-month follow-up.
  • Approved tape: Original 3M hypoallergenic tape
  • Only white or skin-colored fabric tape
  • Avoid transparent plastic tapes (skin damage risk)
  • Tape change:
    – At least every other day
    – Preferably twice weekly during first 2–3 weeks

Proper tape removal:

• Remove tape in the shower after soaking
• Peel gently from top to bottom
• Wash nose with baby shampoo
• If skin feels sticky, clean gently with micellar water
• Perform massage as instructed for 15 minutes
• Apply new tape correctly
• Swelling after showering during the first month is normal

2. Anti-Swelling Injections

Injections prescribed by the doctor:

  • On cast removal day
  • Then weekly (intramuscular)

Completing the prescribed course is essential.

3. Nostril Retainer (Nostril Stent)

• Use the prescribed size every night, and during the day if possible.
• Minimum usage: 3 months
• Some patients may need up to 7 months
• Cleaning before use:
• Soak 10 minutes in Betadine
• Rinse with water
• Spray with alcohol
• If it falls out during sleep, secure with a vertical tape strip.

4. Cosmetic Procedures

  • Lip, chin, cheek fillers: minimum 4 months after surgery
  • Nasolabial fold & frown line fillers: avoid
  • Botox: after 1 month

5. Hair Treatments

Hair coloring, keratin, and hair repair are allowed after 1 month. Use a light mask to prevent chemical fumes entering the nose.

6. Dental Treatments

Allowed immediately after cast removal:

  • Whitening
  • Veneers
  • Crowns
  • Composite & laminate

Root or jaw-related treatments:

  • Lower jaw: after 1 month
  • Upper jaw: after 2 months

7. Follow-Up Visits

Follow-up appointments are mandatory.

Visits must be scheduled in advance.

International patients may have online follow-ups.

Nasal healing and final shaping take at least one year, sometimes longer.
